Problem

The Continuous Quality Improvement (CQI) Committee recently reported that despite implementation of a detailed central line care protocol, the rate of central line infections has not decreased over the last 6 months. The committee recommends that a designated interdisciplinary team jointly review the existing protocol in-depth. The team will explore and monitor this situation and submit its recommendations to the CQI Committee within 30 days. How will the team obtain the Continuous Quality Improvement Committee's evaluation outcomes for this program? How did the Committee collect and use baseline data? What type of outcome is measurement of central line infection (care-related, patient-related, and performance-related)? What are some associated indicators for their organization's program to reduce central line infections (see Table 18-1)? Who is population of interest? Who will/should be interdisciplinary team members? What will each team member bring to the whole team's review? What are the benefits of an interdisciplinary team? What are potential disadvantages of an interdisciplinary team? What will you contribute as a staff nurse team member? What factors should they consider when they benchmark their healthcare organization's program to reduce central line infections with other healthcare organizations' programs to reduce central line infections? Can you think of professional organizations that may have practice guidelines or standards of care for central line care? Check out a few outcome information websites (see Table 18-2) for assistance. How will they use the professional organizations' information as they examine the existing protocol, analyze and interpret outcomes data, benchmark, and make decisions about next steps? How will they use a timeline to classify the outcomes that the interdisciplinary team will set (e.g., short-term, intermediate, long-term)? What are examples for each timeline? What types of ethical issues may arise during data collection about central line care? What are potential validity threats to data that are collected? Do you think that nurses will follow central line protocol differently while data are being collected? What types of information do nursing documents need to state? What are your clinical, leader, and researcher roles during outcomes data collection, analysis, and evaluation?
