A 40-50 year-old male comes into an urgent care facility and is complaining of back pain. The patient tells the nurse who is doing his admission and intake assessment that the pain in his back has been hurting bad for about a week and the patient thought he could "tough it out" but just can't do it anymore. When the nurse asked the patient to point to where the pain is at on his body, he points to the side of his lower back kind of right above where his hips are located. When the nurse was asking the patient about any other health changes they have noticed lately, the patient tells the nurse that he has noticed that when he goes to the bathroom to pee there seems to be very little urine and he tells the nurse that it looks really dark. When asked about his health history, the patient vaguely tells the nurse that he has some "pills he takes for his heart that help with his blood pressure" and some "pills to help control the sugars".
