In the ED the initial laboratory values are as follows: AST 530, ALT 325, alk phos 45, total bilirubin 3, INR 5. The patient's mental status deteriorates, and he becomes obtunded. His vital signs include BP 160/110 and HR 58, and his respiratory pattern is irregular. His wife reports that he has taken an entire bottle of acetaminophen over the past 2 days in an effort to relieve the constant severe headache he has been suffering.
