Community Needs Assessment Project
Purpose: The Community Needs Assessment Project is part of the public health content required in baccalaureate nursing programs. The purpose of this asset-based community assessment project is to promote students' entry-level competencies in practicing public health nursing at the system and community levels of nursing practice.
Preparation
For this project, you are encouraged to choose a health-related topic that interests you and affects the health of your community [may be a community in which you live, work, and/or belong]. The following is an overview of the activities you will complete for this project:
Read Schoon et al. (2019) Chapter 3
Choose an issue of interest that affects the health of your community.
Use epidemiologic principles to understand and explain patterns of the occurrence of this issue in human populations.
Use data sets to identify populations at high risk for illness or disability (related to your specified issue). Consider using Policy Map as a resource to gather data.
Assess assets and needs in your community to address the health issue.
Identify a Healthy People 2030 Goal and Objective to address the health issue.
Identify an evidence-based public health nursing intervention to improve health outcomes in this population.
Strategize methods of implementation and evaluation of your specified intervention.

Instructions/Grading Criteria
Introduction
A brief paragraph that is an introduction to the paper. Identify the health issue and your chosen community. What do you plan to accomplish in this paper?
Overview
Provide a case definition for your specified health issue (standard criteria used to identify this issue). What is the significance of this health issue? What is the cost to society (i.e., U.S. expenditure, etc. Book your online assignment help today)? Explain the overall impact of this health issue on society and the importance of reducing the incidence/prevalence of this health issue.
Incidence and Prevalence Data
What populations are at the highest risk of this health issue? Compare national, state, and local incidence and prevalence data for this health issue. Use credible data sources to describe the demographics of the population affected by this health issue, to include demographic factors that may be considered risk factors for this health issue. Provide a rationale for any differences in the national, state, and local incidence/prevalence rates of this health issue based on population demographics, and other information identified. If local statistical data is not known for your health issue, please specify so.
Local data should be supported by information provided by two key informants within your community who are knowledgeable about the specified health issue (see Talking Points for Interview in D2L). Please note that information from key informants is required to obtain points for this section of the paper.
Epidemiologic Triangle Model or Web of Causation Application
Describe this public health issue using factors from the epidemiologic triangle model or web of causation (see Schoon et al., 2019, p. 89-90). Be sure to use evidence-based information as a rationale for your response.
Windshield Survey
Conduct a windshield survey of your community (see Table 3.7 in Schoon et al., 2019, p. 63). Provide an in-depth
summary of findings from the windshield survey.
Community Protective and Risk Factors
Community Protective Factors: Identify strengths/resources within your community to address the issue. Community Risk Factors: Identify conditions, policies, resource limitations, attitudes, etc. that prevent community members from accessing resources and/or prevent optimal health outcomes.
Healthy People 2030
Identify a Healthy People 2030 Goal and Objective related to your specified health issue that is appropriate for your community.
Public Health Intervention
Identify a community or system public health intervention that may be implemented to improve the health of your community. Outline key components of this intervention. Specify your target population, level of prevention, and type of intervention using the Minnesota Public Health Nurse Intervention Wheel. Provide two sources of reliable evidence to support your proposed intervention (i.e. peer-reviewed journal articles, federal agencies, credible research organizations, etc.).
Implementation
Identify [specific] strategies you would use to implement your specified public health intervention. What resources (i.e., funding sources, partners, etc.) would be necessary to successfully implement the proposed intervention?
What are the potential barriers to implementing this intervention? How may you overcome these barriers?
Evaluation
Public Health Intervention: Identify a [specific, measurable] plan to evaluate the effectiveness of your specified public health intervention.
Healthy People 2030: Identify a measurable health indicator for your specified Healthy People 2030 objective. How will you know if this objective was met?
Conclusion
Provide a brief paragraph that summarizes the findings presented in this paper (no new information should be included).

APA formatted paper
