Assessment - Case Management Plan
Learning Outcome 1: Reflect on appropriate ethical and professional standards in case management.

Assessment Task
Develop a case management plan based on stated values and present issues for further program development. You will be expected to provide evidence of the appropriateness of this plan, based on evaluation of other approaches. At the completion of the case management plan, write a 750-word (+/- 10%) commentary on your plan.
Context
Throughout this subject you have explored the social and economic circumstances many people in Australia experience as well as the community service responses to their needs. You have explored the underlying values of different community service organisations as well as current and emerging models of service delivery.
You have also examined the fundamental values that inform a community service professional's practice and how they are expressed in the code of ethics for case management. You were challenged to reflect on your own personal values and how they may align with the values of professional practice.
After becoming familiar with the steps involved in case management and the role of the case manager, you then reviewed the application of case management in different settings and with different population groups.
You have also become familiar with program planning and the particular steps involved in program planning. You have identified and explored the links between case management and program planning and have identified the similarities of processes and approaches. In particular, the continued application of professional values and ethics are seen to inform both modes of practice.
This assessment will provide you with the opportunity to reflect on and apply your learnings and insights to a particular chosen case study. It will provide you with a further opportunity to demonstrate your understanding of the processes of case management and program planning in a "real world" situation. You will do this by selecting a particular client/community to demonstrate the application of case management planning processes and how they need to be adapted to respond to the needs and circumstances of the situation.
Instructions
To complete this assessment task, you must consider the following elements:
1. Choose a particular client group to develop a case management plan from the following list:
a) Persons with an intellectual disability
b) Persons with symptoms of early onset dementia
c) Persons with disability which impacts their communication and mobility and self-care skills and capacity
d) Newly arrived refugees fleeing from a foreign conflict zone with limited English
e) First Nations people living in remote communities who visit capital city health services for regular treatment of their diabetes
f) Alienated young people from lower socio-economic communities at risk of offending
g) Recovering heroin addicts at risk of or experiencing homelessness
h) Persons with schizophrenia
i) Select your own in consultation and approval from your Learning Facilitator.
2. Using the template provided, develop the outline of a 750-word (+/- 10%) case management plan. Then, write a 750-word (+/- 10%) commentary on the plan considering the following questions:
a) What are the contextual issues and challenges which may arise when seeking to work with these clients?
b) What values do your plan incorporate?
c) Consider the suitability of case management planning. How might you adjust these
approaches to make them relevant to the clients' circumstances?
d) How would you go about evaluating the effectiveness of the plan?
e) What particular steps would you need to take to ensure the client voice and client decisions are reflected and respected in the planning process? What might be barriers to this and how might you address them?
The structure of your 750-word (+/- 10%) commentary should include:
• A separate cover page with the subject name and code, your name, the assessment number and title, and your Learning Facilitator's name.
• Introduction (approximately 125 words) which contains a clear statement of the purpose of the case management plan and what you will cover, together with any relevant background information.
• The Main Body (approximately 500 words) which should contain identification of issues and their disassembly into parts, and critical perspective to be applied in analysing the constituent parts.
• Conclusion (approximately 125 words) which summarises the key points of your case management plan.
• An attachment containing your completed case management plan template.
