2. Meds: Metformin 1000mg po BID for T2DM and Olmesartan (Benicar) 40mg po QD for hypertension and rosuvastatin (Crestor) 10mg po QD. The HgbA1c drawn last week was 8.3%.

3. Vital signs: BP 124/80; HR 72, RR 15; T98; BMI 31.7 kg/m2 (Obesity). The patient tells you that she wants to lose weight.
