Case Study - GLOMERULONEPHRITIS
Mrs. Alice Simpson, age 66, is admitted to the medical-surgical unit with a diagnosis of acute glomerulonephritis and a history of hypertensive cardiovascular disease. She is being admitted directly from her physician's office. She had called her physician complaining of flank pain, headache, and bloody urine. Upon arrival in the medicalsurgical unit, Mrs. Simpson verbalizes being upset about having to come to the hospital. "I really don't want to be here, my family is coming to visit next week and I have a lot to do at home." Mrs. Simpson describes having had a sore throat about 10 days previously that she treated by using Acetaminophen (Tylenol) and throat lozenges. She states, "I woke up yesterday and felt really bad with a severe headache. I noticed that my urine was dark and it looked a little bloody." I waited until today to see my doctor because I thought maybe it was because I wasn't drinking enough water." Her vital signs include: T 99 P 92 R 22 BP 162/78.
1. The nurse notes +3 to +4 peripheral edema on Mrs. Simpson's lower legs as well as periorbital edema. Mrs. Simpson's admission weight is 167 pounds, and the client tells the nurse, "I must have gained weight; I only weighed 150 pounds last month." How does the nurse interpret this statement in light of Mrs. Simpson's current problem (Hint: Describe the pathophysiology of the disorder that led to the development of her current signs and symptoms)?
2. What other focused physical assessment does the nurse need to perform on Mrs. Simpson?
3. The nurse establishes which priority nursing diagnosis(es) for Mrs. Simpson?
4. Diagnostic testing results show an elevated Antistreptolysin O (ASO) titer, confirming a post-streptococcal origin of Mrs. Simpson's acute glomerulonephritis.
Treatment/Interventions include: 1) antibiotics; 2) anti-hypertensives and diuretics; 3) and dietary treatments consisting of a 1000 mg sodium diet with protein restriction and a fluid restriction of 1200 ml/24 hours. Mrs. Simpson questions the nurse about the reason for these interventions. Give an explanation and rationale for each intervention.
