The client appears weak and reports that they have not been able to eat or drink in the past three days due to anorexia, and has also felt constipated. Skin is dry, mucous membranes are dry. The child reports a history of heart failure, hypertension, and hyperlipidemia. Breath sounds are clear bilaterally. Medications include lisinopril, carvedilol and digoxin.
