Description - The maintenance of vital record systems is a core responsibility of every state health department in the nation. According to Turnock (2016) "there is no national mandate for uniform reporting of births and deaths. Through a voluntary and cooperative effort with the states, a national model of these records is implemented by the states, stimulated in part by federal grants for a national cooperative health statistics system." (pg 213) He further notes, "coroners and medical examiners report immediate and underlying causes of death through death certificates, which have two parts, one for entering the immediate and underlying conditions that caused the death and a second for identifying conditions or injuries that contributed to death but did not cause death."
