Question: A 3-year-old child has a 4-day history of temperature to 100.5°F (38.1°C) maximum, coughs mostly at night, and thick, yellow-green nasal discharge. Previous medical history is negative for allergies or any chronic disease. Mom reports that his appetite and activity levels are normal and states, "I'm sure he has a sinus infection because his drainage is yellow and green." Physical examination reveals a nontoxic-appearing child, lungs clear to auscultation, no cervical lymphadenitis, moderate nasal congestion, and copious clear postnasal drainage.
