The nurse notes that six days ago, there was a new order for Aciphex; however, the order fell off the record and did not transfer to the patient's medication-administration record. She looks at the progress notes and sees a note from the pharmacist that is dated the same as the new order. The note reads, "MD notified that patient is already on Aricept. No action needed at this time."
