He states he often gets this pain at night and that he has tried taking ibuprofen which has never helped. He has a history significant for hypertension. His vital signs are blood pressure 145/94 mmHg, temperature 98.7 F (37 C), respiratory rate 20 breaths per minute, heart rate 76 beats per minute, and SpO2 98% on room air. On physical exam, there is a soft systolic murmur at the left sternal border. Lungs are clear to auscultation bilaterally. An electrocardiogram shows evidence of left ventricular hypertrophy. A complete blood count and basic metabolic profile are within normal limits. CK(MB) and troponin are negative on three successive draws. What is the the likely mechanism?
