Case: A study was conducted among 299 elderly persons who had agreed to participate in a study on Alzheimer's disease that would include an autopsy upon their death. Persons were evaluated at autopsy for Alzheimer's disease by established pathological criteria. Patients had also previously been evaluated clinically (using various neuro-psychological tests) for the presence of Alzheimer's disease. Suppose the pathological diagnosis at autopsy is considered the gold standard for Alzheimer's disease diagnosis, and the clinical diagnosis is considered a screening test for Alzheimer's disease.

Of 89 persons with a clinical diagnosis of Alzheimer's, 79 are confirmed by pathological diagnosis at autopsy. Of 210 persons without a clinical diagnosis of Alzheimer's, 51 are found at autopsy to have a pathological diagnosis.
