  • How can it be mediated/intervened (what can be or needs to be done to prevent, eliminate or decrease the issue)?
  • What type of preventions/programs/support need to be either be in effect or put into effect to maintain the intervention and see a positive turn in the health issue/problem?

Part 2 - Program

  • What is the program goal? (What results do you want to see with the program you will develop)? Consider numbers and/or percentages. Consider the demographics, socioeconomic status, of social and ecological factors.
  • Who are the recipients or group of individuals that will participate in the program (think about age, sex, race, and other demographics).
  • What interventions/treatments or activities will make up your program?
  • What made you decide the use the above stated interventions?
  • What other factors will need to be in place or considered in order for the program to be successful?

Part 3 - Evaluation

  • How long will your program run in order to get sufficient data to evaluate its effectiveness?
  • What factors/numbers will you use to evaluate the program's effectiveness?
  • What are your baseline numbers or objectives you will compare the effectiveness of the program to?
  • What means will you use to record/report your data and results?
  • What other factors need to be in place for your program to succeed.
