A 19 year old female presented to the Emergency Department in mid-August with fever and altered mental status. Four days prior she was seen in a clinic with complaints of fever, headache, nausea, diarrhea, dizziness, myalgia and fatigue. She was diagnosed with a viral syndrome and sent home with supportive therapy. Two hours prior to her admission, the patient called a friend and asked to stay with the friend because she was not feeling well. When the friend saw the patient, she immediately took her to the ED. The friend provided the history to the Doctor. The friend had returned from Togo three weeks prior where she took doxycycline as prophylaxis, but her level and compliance is unknown.
