The patient reported he constantly feels like he is having another heart attack from stress, chest tightness, difficulties breathing, and loss of appetite. He reported he has gone for days without eating and has lost ten pounds in the past two weeks. He falls asleep easily due to fatigue but wakes up every three hours and ends up getting only 5 to 6 hours of sleep per night. The patient reported he feels sad and guilty but not hopeless, feels overwhelmed, "sick to his stomach", difficulties managing life tasks and racing thoughts. Denies mental health issues but endorses taking antidepressants in the past did not like the way the medication made him feel so he stopped for about a month.
