Problem

A 48 year old transgender woman with history of a prior depressive episode is given venlafaxine by the PCP to help with her depressive symptoms. The PCP calls you for advice because at the follow-up 2 weeks later the patient now is euphoric, has increased energy, racing thoughts, psychomotor agitation, poor concentration and attention, pressured speech, and a decreased need to sleep. These symptoms began after the patient started taking the new medication. She self-discontinued the medication after 2 days, because she felt better; however, the symptoms have continued ever since. What is the patient's diagnosis?
