A disoriented 58 year-old male patient with a history of poorly controlled diabetes mellitus and chronic obstructive pulmonary disease (COPD) is admitted to the hospital. The patient was a smoker for many years and taking steroids for his pulmonary disease. The patient began feeling poorly after feeding the pigeons in central park. On physical examination, the patient is slightly febrile, lethargic, in respiratory failure and complaining of stiffness in is neck. A CBC and lumbar puncture are performed. The total WBC count is 16, 200 with 78% segs. The lumbar puncture is turbid with 2 neutrophils and moderate extra-large gram-positive cocci that appeared budded. An India ink wet prep shows encapsulated budding yeast.
