Case A 45-year-old Hispanic male with a history of HTN, NIDDMII x 7 years presents to the PCP with c/o progressive blurring of vision since his last visit one year ago. Reading street signs while driving seems more difficult. He is unsure of which eye seems worse. He does not wear any glasses or contacts with the exception of over-the-counter reading glasses. He denies any flashes or floaters, diplopia, eye discomfort, or pain. Further questioning reveals that due to some life stressors he hasn't been taking his meds. States he got divorced lately and he has been very depressed. He is also concerned that his blood pressure has been running high. Last few weeks he has been feeling some numbness and tingling in his lower extremity. States he was admitted to the hospital due to DKA 3 months ago. He was also noted to have uncontrolled HTN at that time.
