Initial History:
69 year old male
Complains of excruciating left leg pain
Reports sudden onset 45 minutes ago while sitting on a bench at the shopping mall
Reports that he has previously had occasional left leg aching discomfort when walking long distances
Additional History:
75 pack/year smoking history
Father died of myocardial infarction at age 78
Mother is alive and suffers from hypertension & cerebrovascular disease
Physical findings:
Left leg is cyanotic, distal to the knee, cool to touch
Right leg is pink & warm
Doppler of the left dorsalis pedis and posterior tibialis pulses reveals decreased pulses with faint bruits heard
Diagnostic tests:
Triglycerides = 315 mg/dl (N:<160)
Cholesterol = 353 mg/dl (N:<200)
HDL = 40 mg/dl (N:40-50)
LDL = 165 mg/dl (N:<100)
Urine is positive for myoglobin
