Problem

A 25-year-old white male smoker with Hodgkin's lymphoma presents with an increasing cough and shortness of breath with exertion over the last 3 weeks. He was diagnosed with lymphoma 2 years ago and was treated with local chest irradiation. About 5 months ago, the patient started having fevers, night sweats, and weight loss and presented to his primary care physician. A CT of his chest, abdomen, and pelvis revealed lymphadenopathy above and below the diaphragm, and he was started on ABVD (doxorubicin, bleomycin, vinorelbine, dacarbazine) chemotherapy. He has completed 4 out of 6 planned cycles. He denies any chest pain or leg swelling. An echocardiogram obtained 3 weeks ago revealed LVEF 65%, no wall motion abnormalities, and poor image quality of right ventricular size and function. Today, a physical exam reveals some dry crackles at the lower lobes and no leg swelling. Labs include CBC, WBC 2.3 with ANC 1.2x10^3/dL, hemoglobin 10.4g/dL, and platelets 140x10^3/dL. Chemistry is unremarkable. What is the next step in the management of this patient?
