Question: A 35-year-old woman is admitted with gradual onset over several hours of sharp pain and swelling of her right knee as well as chills. Her past medical history includes sickle cell disease, a cholecystectomy, and a left hip replacement. She has no allergies and takes folic acid and Tylenol with codeine as needed for pain. She does not smoke, drink, or use drugs. She works as an administrative assistant at an agricultural engineering firm. Her temperature is 38.5, heart rate 103, blood pressure 140/74, respiratory rate 16, and oxygen saturation 98% on room air. Her examination is normal except for her right knee, which is erythematous and swollen, with a balottable patella, and pain on active and passive flexion. Aspiration of her knee reveals 50,000 PMNS per ml, and MRI shows changes in her tibia consistent with osteomyelitis. Gram stain and cultures of the joint aspirate are pending.
