A 45-year-old foreign-born male presented to the ED with cough, fever, chills, night sweats, fatigue and weight loss. Prior to migrating to the United states, he spent some time in a refugee camp. The patient's CXR showed cavitation in the right upper lobe. He was admitted to an isolation room and sputa were collected. Sputum results were positive for acid fast bacilli organisms. The patient reports receiving a diagnosis after having similar symptoms about a year ago but cannot recall the treatment or the duration of it.

1. What is the etiological agent? Describe the morphology (shape, arrangement, gram reaction) and cultural properties (O2 requirement, biochemical requirements, motility, etc...) of this etiological agent.

2. Explain the spread of infection (reservoir and transmission) and pathogenicity (at least three virulent factors) of this agent relevant to this case study.

3. Describe the diagnoses (source of specimen, lab tests, and results that confirm your microbe) and treatment (name of Rx, what it targets, why/how does it work against this microbe) for this agent relevant to this case study.

4. Is there a possibility of drug resistance in this case? Explain how it could have developed and what can be done this time to ensure complete recovery and elimination of the microbe.
