Question: The patient care environment is complex and prone to errors. According to the Joint Commission (2018), the most common sentinel events in health care include wrong patient, wrong site, wrong procedure; unintended retention of foreign body; inpatient suicide; operative/postoperative complications; and perinatal death/injury. In each of these categories, harm is done to a patient. Using one of these categories of sentinel events, describe a situation where there is a primary and secondary victim.
