Problem
Prompt: You have been working with a 55-year-old man, Peter in outpatient services for the past year. When you started seeing him you knew of his history of opioid and alcohol addiction. Prior to meeting with you, he had been able to detox from opioids and has remained opioid-free for close to 4 years. In sessions, he acknowledges that his struggles with alcohol have continued. Recently he cancelled two appointments with you. When you were finally able to connect with him by phone you learned that he had been recently laid off. He had worked in an engineering job for the past four years. In this job, he made six figures. But he lives in an impoverished area due to child support and alimony payments. Prior to this job, he was homeless for close to 1 year. During this time period, one of his family members gave him a car and that seemed to be the impetus to work on his health. On the phone, you notice that he is rambling and isn't making much sense. When you ask if he is okay he shares that he has been on the couch watching TV and feels lonely. In sessions, you have learned of Peter's traumatic childhood. You have a session set for tomorrow.
It is important to note that his wife of 16 years divorced him five years ago. She filed a restraining order and to this day refuses to allow him visitations with their two girls. The ex-wife also has a history of opioid and alcohol addiction. When their oldest daughter was 5 years old, the ex-wife Tammy passed out on the couch. Peter was traveling for work and unable to assist. The 5-year-old ran to the neighbors who called the police. Tammy had a blood alcohol level of .3 and may have died if her daughter did not get help. She entered detox and rehab and claims she is now sober.
