A 53-year-old man presents to you for epigastric discomfort and burning for the last 3 weeks. He has been taking 600 mg of ibuprofen 4 times per day for worsening lower back pain during the last 2 months.

Discontinuation of the nonsteroidal anti-inflammatory drug and a trial of a proton pump inhibitor (PPI) improve his symptoms, but the treatments do not completely resolve his symptoms.

Upper endoscopy is performed and a large ulcer is noted in the gastric body. Rapid urease test and Giemsa staining are both negative for Helicobacter pylori.
