(CASE STUDY)

A long term care facility admitted Mrs. L., age 71, six (6) weeks ago. She had been discharged from the hospital following an episode of congestive heart failure. Mrs. L. has had congestive heart disease for five years, and also suffers with diabetes. Her husband died six months ago and her only daughter lives in another state.

One morning when the practical nurse student enters Mrs. L's room to do an assessment Mrs. L., states, "Since Jason died nothing matters anymore." During the assessment, the student nurse asks Mrs. L. about her activities. She states that she has recently had a weight loss of 10 pounds because she's "just not hungry". Mrs. L. also states she is finding it difficult to sleep at night and is often so tired that she doesn't feel like leaving her room or participating in any of the recreational activities that are provided at the facility. Mrs. L. hasn't talked to friends recently and speaks to her daughter only when she calls. She tells the student nurse that she really doesn't want to talk to anyone. She states, "It hurts too much. I know it will soon be over." The student responds, "What makes you think this?" Mrs. L. states, "I have only a few days to live."
