A 36-year-old post op patient is given acetaminophen 325 mg with codeine 30 mg (Tylenol with codeine # 3), two tablets PO Q3hr. However, pain is not controlled. The provider prescribes hydrocodone 5 mg with acetaminophen 500 mg (Lortab 5/500) Q 4 hr. Even with the new prescription, the patient continues to complain of pain ranked 8 on a 10-point scale. Vital signs: R 24, HR 110, BP 140/85. What is your assessment of this patient's pain and what are reasonable analgesia goals? Is the choice of acetaminophen with hydrocodone appropriate? What would you suggest? After several doses of the opioid, the patient complains of constipation. What can be done for opioid-induced constipation?
