What is the diagnosis for Mrs. W is a petite, pretty, well-dressed 28-year-old woman who appears to be deeply ashamed of herself and uncomfortable talking about her problems with the interviewer. On examination, her temperature is 99.8F, her blood pressure is 140/90, and her pulse rate is 104. She reports insomnia, a sensation of bugs crawling on her, and a feeling of anxiety, and she appears tremulous. Mrs. W has not had a drink for the previous 12 hours before the interview and is currently experiencing and extreme craving for alcohol. She talks about feeling low self-esteem and hopelessness about whether it will be possible for her to stop drinking; however, she is able to be quickly reassured when reminded of her ability to stop drinking during pregnancy and about her mother's success at remaining abstinent. Mrs. W has been able to function, has not generally felt depressed, and reports that she has several close friends at work who think of her as being a lot of fun.
