Assignment:
J.S., a 60-year-old man with chronic obstructive pulmonary disease (COPD) related to cigarette smoking. He has been on home oxygen, 2 L oxygen by nasal cannula, for several years. Approximately 10 months ago, he started chronic oral steroid therapy. His current medications include an ipratropium/albuterol. (Combivent) inhaler, beclomethasone (Beclovent) inhaler, dexamethasone (Decadron), digoxin (Lanoxin), and furosemide (Lasix). He is in the hospital for exacerbation of his COPD with symptoms of fluid overload and edema noted to his hands and feet. Upon assessment, you note he is obese, but has skinny arms and legs. Noted reddish purple striae on abdomen and thighs, and a very round face. Pulses are full and bounding. His wife states he has been moody and acting depressed lately. He has a wet cough. Current VS: HR 98, BP 173/101, RR 24, O2 sat 89%, Temp 99.9
