Peter Kelly is a 73-year-old man with long-standing diabetes. Today, he presents to the emergency department in a coma. You review his past history and note that he was diagnosed with type 2 diabetes about 12 years ago. He experienced a stroke about a year ago and has been living in a long-term care facility since then. His care providers indicate that his blood glucose levels are monitored daily. The levels are typically around 160 to 200 mg/dL, but these have been steadily climbing over the past 2 weeks. He is on an oral glycemic agent. He does not exercise due to residual weakness from the stroke, and he does have frequent visitors who like to bring him hard candies.

He had an appointment scheduled this week to meet with his primary care physician to evaluate a leg ulcer that is possibly infected. The care providers indicate that his urine output has increased over the past 2 weeks. They are not sure if his fluid intake has increased. When they went into his room this morning, he was unresponsive. His current blood glucose level is 721 mg/dL. His urine is positive for glucose and negative for ketones.
