Case Study: Medical Record

History: The patient DG is a 50 year old male presenting to the dermatology clinic with concerns about pruritus located at the medial aspect of his left eyebrow secondary to dermatitis. Patient also showed dermatologist a tender, discolored thickened nail of the left hallux. The patient's medical history is also significant for basal cell carcinoma, primarily of the ears as well as chronic psoriasis primarily of the forearms bilaterally.

A diagnosis of psoriasis was made. Eczema, scabies, and tinea were considered in the differential diagnosis. An emollient cream was prescribed. Additionally patient was diagnosed with onychomycosis for which he was given an additional prescription for an oral anti-fungal drug.
