Jack, a two-and-a-half-year-old boy is brought in by his parents. His parents ask for an antibiotic prescription right away. Jack's mom is sure that he has an ear infection because he has had a runny nose for about a week then started getting fussy and pulling on his left ear. According to her, the last time he had these symptoms he was diagnosed with an ear infection. He has not had any ear discharge. Jack suffered from a mild upper respiratory tract infection (URTI) which he caught from daycare for the last week but was not febrile until last night. He has not had any diarrhea, or vomiting. The only relevant point on past history is a previous AOM at the age of one which was treated for ten days with an unknown antibiotic. He has no allergies.
On physical examination, he is awake and playful. He has a runny nose with clear discharge. His lungs are clear to auscultation, there are no other abnormal findings on examination. There is no lymphadenopathy. On otoscopic examination, you see a bulging and erythematous left tympanic membrane. His right tympanic membrane is normal. His VS include HR 110, T 100.9, RR24, BP 90/60, Ht. 36 inches, Wt. 31 pounds.
You diagnose him with acute otitis media and want to give him prescriptions for his pain/fever and an antibiotic.
