Mrs Mary Windsor is a 70-year-old Aboriginal woman of the Gadigal people of the Eora Nation. Mrs Windsor is a widow after losing her husband George 10 years ago, and now lives in Redfern Sydney, with her daughter Jesse Armstrong (37 years old), her son-in-law Tom Armstrong (40 years old) and her two grandchildren Billy (6 years old) and Maisy (4 years old).
Mrs Windsor has been retired now for 20 years after working as a teacher's aide at the local primary school.
Mrs Windsor has been diagnosed recently with small cell carcinoma of the right upper lobe of the lung. Mrs Windsor is breathless after activities of daily living and is not seeking any medical treatment for her tumour. She has a history of smoking for approximately 40 years but has been a non-smoker for the past 2 years.
Mrs Windsor has recently been showing some signs of vascular dementia and has some periodic episodes of confusion and incontinence. She suffered an Ischemic stroke 5 years ago and recovered well maintaining most functions of daily living.
Mrs Windsor has some sight impairment due to bilateral cataracts and wears prescription glasses when awake. She has some hearing impairment but does not use hearing aids.
Due to Mrs Windsor's episodes of confusion and recent incontinence, Jesse is finding it more difficult to care for her mother's needs, combined with the demands of her work and family. Jesse is wanting to find out what support is available for her mother, and ideally would like her to stay at home for as long as possible.
Mary can manage her own grooming and hygiene but needs reminding to use the toilet and maintain her oral health care. Mary requires supervision during the day due to her increased confusion and wandering.
Mary's daughter, Jesse, wants to receive some home support. Develop a plan for home and community support for Mary based on her Aged Care Assessment Team package.
Mary has been assessed as a level 2 care needs and she has no income or assets.
