The drug reference provides the following information: Route/Dosage PO (Adults): 0 mg once daily initially, may be increased in 1 wk to 40 mg/day (maximum dose); CYP2C19 poor metabolizers or concurrent use of CYP2C19 inhibitor-Not to exceed 20 mg day. PO (Geriatric Patients): 20 mg once daily (do not exceed dose of 20 mg/day in patients >60 years). What is an appropriate dose after 1 week?
