Assignment: Part 1: A 56-year-old patient with newly diagnosed stage 1 hypertension has been referred to you for counseling regarding lifestyle modifications. He is married, with four children - two in high school, two in college. His job as a senior vice-president for a major retail chain requires that he work long hours and frequently eat at restaurants. He smokes two packs of cigarettes a day, has a body mass index (BMI) of 29 kg/m2, and a waist-hip ratio of 1.6. He usually drinks one to two dry martinis to relax after he gets home from work.

1. How would you develop a realistic plan to help this patient reduce his blood pressure and prevent complications?

2. Which risk factors would be among your top two or three priorities for this patient, and what interventions or recommendations would you provide for modifying these?

Part 2: Therapeutic communication is important to ensure patients are part of treatment planning.

1. How would you evaluate a patient's ability to understand your instruction and their current knowledge base about their problem? What characteristics of the patient would be helpful?

2. How would you be certain that the patient understands your medication instruction?

3. What methods of therapeutic communication would be useful in advanced practice?

Part 3: Research the range of contemporary issues teenagers face today.

Choose one issue (besides teen pregnancy) and discuss its effect on adolescent behavior and overall well-being. Include the following in your submission:

1. Describe the contemporary issue and explain what external stressors are associated with this issue.

2. Outline assessment strategies to screen for this issue and external stressors during an assessment for an adolescent patient.

3. Describe what additional assessment questions you would need to ask and define the ethical parameters regarding what you can and cannot share with the parent or guardian.

4. Discuss support options for adolescents encountering external stressors. Include specific support options for the contemporary issue you presented.

Part 4: Topic: Infection Control and Prevention

A PICOT question starts with a designated patient population in a particular clinical area and identifies clinical problems or issues that arise from clinical care. The intervention used to address the problem must be a nursing practice intervention.

1. Include a comparison of the nursing intervention to a patient population not currently receiving the nursing intervention, and specify the timeframe needed to implement the change process (Infection Control and Prevention).

2. Formulate a PICOT question using the PICOT format (provided in the assigned readings) that addresses the clinical nursing problem.

a. Describe the problem in the PICOT question as it relates to the following:

• Evidence-based solution

• Nursing intervention

• Patient care

• Health care agency

• Nursing practice

3. Identify the clinical problem (Infection Control and Prevention) and how it can result in a positive patient outcome.

Part 5: Topic: Infection Control and Prevention

Review your problem or issue (Infection Control and Prevention) and the cultural assessment.

1. Consider how the findings (in articles or academic bibliography) connect to your topic and intervention for your capstone change project.

2. Write a list of three to five objectives for your proposed intervention. Below each objective, provide a one or two sentence rationale.

You must include in the objectives the topics that this research will address:

• Cleaning and disinfection

• Standards precaution

• Cross-contamination

3. Provide a rationale for how your proposed project and objectives advocate for autonomy and social justice for individuals and diverse populations.

Part 6: You will develop your personal philosophy of teaching in a written statement, articulating your beliefs about important educational theories and practices.

Your teaching philosophy should be based on what is meaningful to you in your approach to teaching.

There is no right or wrong way to write a personal teaching philosophy. However, for the purpose of this assignment, please follow the assignment guidelines below to connect your beliefs, goals, and strategies into a coherent approach to help students learn and grow.
