There is a new bug is sweeping the area, the symptoms are flu-like, may involve the GI and CNS systems, possible encephalopathy in a few cases. In the extreme, there have been reports of incidences of anaphylaxis upon second exposure. What is known is that it appears to be systemic. Mucus secretions from an individual that has successfully cleared the infection can reduce the infectivity of the "bug" in culture. The bug can be cultured. Mucus secretions from a host that is naïve, does not affect the virulence of the "bug". Serum from individual's displaying the array of symptoms is available.

We need diagnostic tests:

1. What immunoglobulins are you going to look for and why? Remember, we know it is systemic, the mucus has neutralizing properties and there appears to be an anaphylactic response possible:

2. Do you think the ability of the mucus to block infectivity is adaptive or innate? Explain your reasoning.

3. Why do you think there is anaphylaxis?

4. How could you enrich serum for Ig specific to the bug?

5. How could you prepare "bug" antigens?

6. Design assays for all the Ig's you listed in question 1. (include controls needed!!)

More space to diagram your Ig assays.

7. How would you determine if there were NK or Tc involvment in the clearing of the infection?

This assignment is about different facets of immunology. The paper contains the details of different immunoglobulins, various assays to identify those, putting forth various mechanisms like anaphylaxis, mode of preparation of bug antigens, enriching serum for Ig specific to the bug and involvement of NK or Tc cells in clearing infection in the form of question and answer format. The identified immunoglobulins were A, G, and E, innate immunity comes into play while blocking infection by mucus, anaphylaxis is a hypersensitive reaction, various methods like chromatography, affinity ligand antigen interaction were used to enrich serum, ELISA was used to measure and identify immunoglobulins, and role of NK and Tc cells were discussed. It has been prepared on MS word.

To preface this exam I need to make a few comments. Please don’t panic on me, I know this may seem a bit vague or broad, but I want to allow you to use your background to formulate answers. Since most of you have very different foci and backgrounds, I have to allow you a broad target. I do not believe that there is a single correct response, I believe that there are many, many ways to respond and be within the ballpark or close to the target. What I am looking for is your ability to apply your knowledge of immunology to a clinical laboratory problem……..immunodiagnostics. Granted, most of you will not be designing your own assays, but to problem solve the “kits” we use, you need to be able to break them down and understand each part, what better way to demonstrate that you can do that than to build your own. This is also not something you can look up in the book. It needs to be thought out and logically presented. There are clues. So……..here goes:
