Autonomy Mr. Shara an 84-year-old has evidence of memory loss and periodic confusion for about one year. He has been able to do personal care at home and has been functional on no medications. One night he came to the bedroom with a straight razor and told his wife to stay still so he could slit her throat. She was able to escape and called her son who helped calm him down. At the advice of her husband's doctor she called a geriatric psychiatrist who immediately hospitalized him and started him on Haldol. After a day in the hospital Mr. Shara became incontinent, could not walk, was drooling and had to be restrained. Mr. Shara was scheduled to be moved to a nursing home, but Mrs. Shara wanted to care for him at home. After much persistence she was able to convince the psychiatrist to discontinue the medications and his condition improved and he was able to go home. Although he is still confused at times, he remains functional; his wife has made sure all sharp or dangerous objects are kept hidden.
