Case: The antibiotic gentamicin causes concentration-dependent kidney and hearing toxicity, which may be irreversible. However, its capacity to kill bacteria also depends substantially on the peak concentration that is reached after an intravenous bolus dose. Thus, peak plasma concentrations are measured to ensure that the peak is high enough. Trough concentrations are then measured to ensure that most of the dose is eliminated before the next one is given. A commonly-employed target concentration for the peak is around 6 mg/L. The trough concentration before the next dose will commonly be around 0.5mg/L. The population average volume of distribution for gentamicin (Vd) is 0.8 litres per kilogram of body weight.

What dose needs to be given to a 70kg woman to achieve a peak concentration of around 6mg/L in plasma?

If you gave her a first dose of 300mg and then found that the measured peak concentration was 4mg/L, what would you calculate her actual volume of distribution to be?
