Case: A young man walks into ER very quiet & sleepy. He walked to ER from his friend's house (a mile away). His pulse is 134, respirations 36, BP 122/88 and temp 96.4°. His color is ashen, and his skin clammy. There is an empty inhaler in his pocket & a half empty packet of cigarettes. He states he couldn't afford to refill the inhaler. The nurse replies "You must not need it then." The young man's jacket smells of wood smoke and stale beer. He says in a quiet broken sentence that he was at a campfire.
The nurse examines this young man. She interprets his sleepiness as a sign that he has had too much to drink. She assumes that his vital signs are related to anxiety about his parents finding out that he has been at a party. She listens quickly to his bases and finding no breath sounds comments that he has shallow breathing. She reports to the next nurse that he just had too much to drink & just needs to sleep it off. She comments that she couldn't find the oximeter but "He's fine As the new nurse goes into the room she finds that the young man has been arrested.
