Case Study: The summary report (see Summary Report Template, linked in this activity assignment in Module Four of your course) is a document created to present to the provider after the audit of its charts has been completed. This document will identify the charts that were audited and the findings of each chart. It will give the provider a score for the audit, which indicates its documentation compliance rating. The report will also identify any problem areas found during the audit and make suggestions for improving the score if needed. This document will provide the educator with valuable information when talking with the provider. It will also give the administrators of the facility detailed information to use to determine educational needs in the facility.
