Pt is a 15-year-old female. She has a diagnosis of "anxiety and mood disorder." She has tried: Prozac, Celexa, Lexapro, Zoloft, hydroxyzine, trazodone, Risperdal, Lamictal, and Seroquel. She has a history of and continues to self-harm (cutting), displays an unstable affect, fluctuates between extremes of idealization and devaluation, has a persistently unstable sense of self, and feels chronically "empty." You recognize another medication may be minimally helpful. What do you review with the patient or family, and what are your clinical recommendations? What evidence supports these decisions?
