Eight weeks following his original collapse, Leo again visits his physician, now complaining of respiratory problems. With a stethoscope, Dr. Tong detects some wheezing and inspiratory rales and orders a chest X-ray. The X-ray films show a markedly enlarged cardiac silhouette and a generalized haziness at the base of Leo's lungs.

Each student should select one of the following to discuss. Threads have been created where students may post their answers. Students also are expected to respond to a post by another student.

  • What is wheezing? inspiratory rales?
  • What do the lung sounds and haziness suggest is happening in Leo's lungs?
  • How might these lung symptoms relate to Leo's defective valve?
  • Why is the heart enlarged? (Hint - how might the heart attack have affected Leo's normal stroke volume?) Which chamber of Leo's heart is most likely enlarged?
  • What is the clinical name for Leo's condition? (Think about the condition of both his heart and his lungs.)
  • What symptoms might one see if it had been the other side of Leo's heart that had been affected by his MI?
  • Besides causing some damage to the function of Leo's valve, how else might the MI have contributed to this clinical condition in Leo?
