Target Population

Name of the agency==== "All Faith Food Bank."

The target population for this project is retired individuals, this results in stress and insecurity regarding one's financial situation, which can contribute to chronic diseases. Also, this group population is concerned about their ability to obtain adequate nutrition. The lack of access to nutritious food and the inability to afford the food they need to maintain their health may indicate that they do not have regular access to nutritious food. Lack of understanding regarding nutrition and how to maintain a balanced diet is another barrier. This is a concern since a significant portion of the population in question is unaware of the significance of maintaining a healthy diet and may not know how to achieve this goal.

ANSWER THE FOLLOWING QUESTIONS:

Question 1: Why did you choose this particular group?

Question 2: Statistics (demographics) about this specific group (describe the group, not the individuals)

Question 3: Describe the health concerns or interests of this group.

Question 4: Discuss the broad topic you are addressing for your project and why this topic is necessary for this target group.

What observations or interactions have you had with group/agency contacts that support the selection of this particular topic for your project? What evidence do you have that this is something to be addressed with this specific group?

What determinants of health have you noted or observed that relate to the topic, these should include physiologic, behavioral and/or social? (For instance, if you are educating on nutrition, what factors influence the person's ability to change to a healthier diet or not, and how does that affect their health?)
Plan

What is the goal the group has for their time with you during this course?

Who will you consult with to ensure the project has input from multiple perspectives - agency staff, at least one other discipline, etc.? Be specific.
