Case Study - Mrs. C. is an 82-year-old widow. She moved from her long-time home residence into a one-bedroom apartment about six years ago. Her primary insurance is Medicare. She maintains her apartment in the 55+ community, and does all her errands without assistance, all of which she is very proud. She reports that she has occasionally fallen, possibly because of some degree of decreasing vision, especially when moving from direct sunlight into shadows and when walking at night. The only medications she takes are an analgesic for pain management of osteoarthritis, an antihypertensive agent, and a daily vitamin and cranberry supplement.
Today she drove herself to her annual check-up with nurse practitioner Kelly at her primary care doctor's office. The clinic is a part of a new practice of "integrative medicine". They emphasize the role of prevention, including screenings, and are big in promoting health. Think like a health promotion nurse.
Gordon's functional health patterns is a foundation for nursing assessment and diagnosis. Complete Gordon's Functional Health Pattern Assessment on Mrs. C., as if you are the nurse practitioner. Answer the following questions with the eleven functional health patterns in mind. Utilize the pattern examples in chapters 6-8, on health promotion and the individual (Mrs. C), the family (Mrs. C's family), and the community (where Mrs. C. lives). Think critically and holistically.
Choose at least five functional health patterns to assess in answering the first three questions.
1- What data will Kelly collect to assess Mrs. C.'s health? Why?
2- What questions will Kelly ask to evaluate Mrs. C.'s health status? Why?
3- What risk factors will Kelly most likely evaluate? Why?
4- What screening tests will Kelly suggest? Why?
5- Propose 3-5 probable nursing diagnoses for Mrs. C. Remember to think health promotion and disease prevention in your diagnoses.
6- Propose 1-2 interventions for each diagnosis. Use the following template if desired.
The nurse practitioner (Kelly) has completed the Functional Health Pattern Assessment on Mrs. C. and she wants to complete the plan of care for continuing to promote her health and prevent further illness.
Kelly asks Mrs. C. about her stress level and how she manages stress. Mrs. C discusses she is stressed due to worsening vision and fear of falling. She also fears not getting help when she needs it. Her daughter and family recently moved to Eastern WA and now live 5 hours away instead of living within 30 minutes of her apartment. Her grandchildren used to visit several times a week, and phone calls and "Face Time" aren't quite the same. Her friends don't visit as often due to covid fears and one close friend died from covid last year.
7- What are some of the physical symptoms/warning signs of stress? Include behavioral, emotional, cognitive, and relational warning signs of stress. (List at least 10.)
8- Suggest at least five coping strategies Mrs. C. could employ to help manage her stress. (Incorporate exercise if you can. Think "outside the box" too.)
