His clothes are wrinkled and look like they could using a washing. He's in need of a shave, a haircut, and maybe a shower. He reaches out his hand to shake yours. He then begins to tell you that he lost his job and his health insurance. He is a diabetic and can't afford his Novalog Insulin Pen and he doesn't know what to do.
