Problem: CASE STUDY Mrs. BSS, a 44 y/o African American who was admitted with cholecystitis. After undergoing an open cholecystectomy, she was admitted to the surgical floor. She has an NG tube to continuous low suction, a 22 g peripheral IV and a large abdominal dressing with scant amount of blood. She has a history of pre-diabetes, obesity, depression. Previous surgical history of C-section x 2 and breast enlargement. She has MD orders as follows: Advance diet to low-fat diet as tolerated D5.45%NS with 20 meq KCl at 100 mL/Hr Incentive spirometry q2 hrs OOB in chair and ambulate with assistance as tolerated PM, ambulate in AM 100 feet Morphine 2mg IV, q4hrs, PRN for pain Reglan 5mg IV q day Zofran 4mg IV PRN for nausea Zoloft 50mg PO daily Labs in AM, CBC, electrolytes, fasting glucose Are these orders appropriate for BSS? What is your rationale? What gastrointestinal complications might result from one of the medications listed in BSS' orders? Four hours after surgery the CNA reports: BP 148/77, HR 118, respirations 24, temperature 101 degrees Fahrenheit. Based on these vital signs, what could be happening with Mrs. BSS?
