Problem: Acute Coronary Syndrome

A 36-year-old female client is recovering from a percutaneous transhepatic cholangiogram (PTC) via interventional radiology this morning. The client has a history of cholelithiasis with reocclusion of the cystic duct after an endoscopic retrograde cholangio-pancreatography (ERCP) 2 months ago. The client is alert and oriented. Her sclera is yellowed by jaundice and she rests comfortably under several thick blankets. Her medical history includes hypertension and hyperlipidemia. She is 5 foot 4 inches (1.6 m) tall and weighs 95 kg (209 lb). Her current prescriptions are atorvastatin 40 mg/day and metoprolol 25 mg/day.

The client awakes abruptly and calls for the nurse. She reports feeling short of breath with pressure in her back between the shoulders. She also states, "I feel like I am going to vomit."
