Assignment:

For the first part of this assignment, each student must go out into the community and interview a "patient". The "patient" may be a friend, relative, patient, or anyone else who agrees to participate. No personal identifiers will be used in the project. The "patient" must currently take five or more medications. At least two of the five medications must be prescription medications, which are taken on a regular basis, not as needed. The medications will include all pills, injections, eye/eat/nasal drops, or topical applications. The medications must also include all over-the-counter, herbal, or supplements.

The paper will include a schedule of all the medications the patient takes.

Things that must be listed:

Patient initials, age, gender, and current health conditions.
Medication name (generic), the dose strength, and number of pills/drops/amount of liquid per dose.
Mechanism of action/the drug's intended effect.
What condition is the patient taking the medication for?
Medication instructions: take with food, empty stomach, morning/evening, medications/foods to avoid, etc.
What time does the patient take the medication (this may be different than the medication instructions)?
