You are the RN working 0700-1900 on a medical-surgical unit. Your patient, Mr. D is two days post-op after an emergency appendectomy. Mr. D had never been seen in this facility previously and was reportedly traveling through town when severe RLQ pain brought him to the Emergency Department. No other history is provided.
His 0700 Vital Signs were as follows: HR 89, T995 F, BP 139/89, RR 15, 02 Sat 96% on 2L
However, during the 0900 med pass you noticed that his hands were showing tremors when he took the med cup and he appeared more agitated and restless that when you took care of him yesterday.
At 1100 his Vital Signs your CNA called to share that Mr. D's HR was 101 and temperature was 101.5 F
Initially, you suspect this patient has sepsis.
